Key responsibilities & impact

Deliver comprehensive virtual care, including patient assessments, condition management, medication review, and appropriate escalation, while ensuring compliance with telehealth regulations and licensure requirements.
Collaborate with multidisciplinary teams and utilize telehealth workflows to support coordinated, continuous care and maintain accurate documentation of patient encounters.
Educate patients on preventive care, disease management, and healthy lifestyle behaviors to improve outcomes and encourage active participation in their health.
Provide compassionate, culturally sensitive care while maintaining professionalism, strong communication, and sound clinical judgment in all interactions.
Adhere to clinical guidelines, regulatory standards, and organizational policies while participating in quality improvement initiatives.
Ensure accurate, timely documentation, uphold patient privacy, and maintain high standards of safety, ethics, and confidentiality.

Requirements

What you’ll need

Active, unrestricted current NY licensure as a Provider in the state in which you will be providing care for patients.
License must be clear of any clinical and/or board practice restrictions.
NY State only: Minimum of 3600 hours of practice experience, or with 3 years of clinical experience (autonomous provider), in a primary care or longitudinal care setting
Board Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner or Physician Assistant
Obtain autonomous provider status as required by state you are licensed with currently within six months of meeting eligibility requirements.
Active Drug Enforcement Administration (DEA) license.
Current Basic Life Support Certification for health care providers.
Proficiency in electronic medical record (EMR) systems.
