About the role

  • Must be Bilingual in English and Spanish
  • Support the Chronic Care Management Program by providing both clinical and administrative assistance to the care delivery team
  • Serve as a primary point of contact for patients by conducting intake and confirmation calls, communicating provider‑reviewed results and care plans, and supporting initial visit preparation
  • Coordinate scheduling and rescheduling for in‑person and telehealth visits, follow up on missed appointments, and ensure continuity when provider availability changes
  • Track patient admissions, discharges, and activity across hospitals, skilled nursing facilities, and other care settings; document updates in the EMR and notify the care team
  • Support transitions of care by obtaining and uploading medical records, coordinating respite or long‑term care stays, and scheduling return‑to‑residence visits as directed
  • Provide day‑to‑day operational and clinical support to primary care teams, including managing the clinical inbox, processing medication refills, prior authorizations, and insurance‑related forms

Requirements

  • High School Diploma or equivalent
  • Current certification or registration (CMA, RMA, CCMA, or equivalent)
  • Basic Life Support (BLS) certification
  • 1+ years of experience in a clinical, healthcare, primary care, or outpatient clinical setting
  • Familiarity with EMR systems and basic clinical documentation
  • Proficiency with Microsoft Office Suite, including Outlook, Word, Excel, and Teams
  • Customer Service experience
  • Ability to pass background check upon hire and throughout employment
