About the role

Key responsibilities & impact

Contact caregivers prior to or during scheduled visits to confirm arrival at the patient’s home.
Ensure caregivers have the current and appropriate patient Plan of Care.
Email or securely transmit care plans and related documentation to caregivers when necessary.
Document all communication, follow-ups, and care coordination activities accurately in the EMR system.
Escalate patient care concerns, missed visits, or compliance issues to the appropriate clinical supervisor.
Maintain HIPAA compliance and patient confidentiality at all times.
Support continuity of care and timely coordination between field staff and office personnel.
Assist with basic care coordination and administrative nursing tasks as assigned.

Requirements

What you’ll need

Registered Nurse with New York License - USRN - NY
Fluency in both Spanish and English required
Minimum 1–2 years of experience in home health, care coordination, or clinical scheduling preferred
Strong organizational and time-management skills
Proficiency with scheduling software and electronic health records (EHR) systems
Excellent interpersonal, problem-solving, and multitasking abilities
