About the role
Key responsibilities & impact- Contact caregivers prior to or during scheduled visits to confirm arrival at the patient’s home.
- Ensure caregivers have the current and appropriate patient Plan of Care.
- Email or securely transmit care plans and related documentation to caregivers when necessary.
- Document all communication, follow-ups, and care coordination activities accurately in the EMR system.
- Escalate patient care concerns, missed visits, or compliance issues to the appropriate clinical supervisor.
- Maintain HIPAA compliance and patient confidentiality at all times.
- Support continuity of care and timely coordination between field staff and office personnel.
- Assist with basic care coordination and administrative nursing tasks as assigned.
Requirements
What you’ll need- Registered Nurse with New York License - USRN - NY
- Fluency in both Spanish and English required
- Minimum 1–2 years of experience in home health, care coordination, or clinical scheduling preferred
- Strong organizational and time-management skills
- Proficiency with scheduling software and electronic health records (EHR) systems
- Excellent interpersonal, problem-solving, and multitasking abilities
