About the role

Key responsibilities & impact
  • Coordinate admission, transfer, and consult processes for emergent, urgent, and elective patients.
  • Process patient acuity and apply clinical decision-making to facilitate patient flow.
  • Document and prioritize patient information for accepting physicians, nurses, and healthcare workers.
  • Maintain current knowledge of departmental procedures, protocols, and communications.
  • Assign transfer priority levels for patient triage and transportation.
  • Maintain Transfer Center data and provide feedback to referring facilities.
  • Screen admissions for appropriate level of care, patient status, and bed type.
  • Apply Medical Necessity criteria.
  • Educate nurses, physicians, and ancillary staff on level-of-care and admission/transfer issues.
  • Serve as liaison among physicians, patients and families, hospital staff, patient-care areas, and outside agencies.
  • Monitor compliance with regulatory requirements, EMTALA, ethics and compliance policies, and quality initiatives.
  • Track and trend barriers to patient flow; recommend process improvements and develop action plans.
  • Escalate issues to Transfer Center leadership and facility administrative leadership.
  • Coordinate with the Bed/Capacity Coordinator on timely transfers, bed assignments, and accurate patient information.
  • Maintain accurate information in the Electronic Health Care Record.
  • Participate in employee orientation, ongoing training, staff meetings, and continuous quality improvement projects.
  • Handle inbound calls and make outbound calls to meet departmental productivity goals.
  • Provide superior customer service and support Transfer Center key performance indicators and Access Point Vision goals.

Requirements

What you’ll need
  • Graduate of an accredited school/college.
  • Must hold applicable current RN licensure in the state of residence.
  • Must maintain current nursing licensure and complete renewal and continuing-education requirements.
  • Three years of related clinical experience in an acute-care setting; ED or Critical Care preferred.
  • Ability to communicate effectively in oral, written, and electronic formats.
  • Ability to multitask while attentively listening and typing more than 35 words per minute.
  • Must meet internet speed requirements: upload 50 or more; download 100 or more.
  • Knowledge of human growth and development, assessment, treatment, and care for patients of all ages served.
  • Competence applying Medical Necessity criteria.
  • Knowledge of regulatory requirements, EMTALA, ethics and compliance policies, and quality initiatives.
  • Experience with electronic health-care records and electronic resources.
  • Preferred Compact License.
  • Must meet current nursing licensure requirements; expired licenses prohibit scheduling.
  • Associate’s/bachelor’s degree preferred.
