About the role

Key responsibilities & impact
  • Support and assist Case Management/Care Coordination/Transition of Care and resource utilization efforts throughout the LifePoint System to improve care processes and patient outcomes.
  • Collaborate and coordinate with the National Medical Director to develop and implement the Hospital Support Center (HSC) care management strategy for the Company and respective divisions of facilities.
  • Evaluate pertinent Case Management metrics within division facilities, identify opportunities for improvement, and facilitate actions to address the priorities at the facility or division level.
  • Conduct onsite Case Management programs assessments with facilities and potential new affiliates; Work with hospital Case Management and leadership teams to track, trend, educate/train, monitor, and develop effective corrective action plans
  • Maintain open communication with Health Support Center (HSC) division and department leadership team regarding performance of individual hospitals, gathering feedback for further improvement opportunities and strategies.
  • Collaborate with internal HSC departments to develop and present targeted education to optimize compliance with regulatory and payer requirements which promote high-quality, cost-effective care.

Requirements

What you’ll need
  • Education: Bachelor’s Degree in Nursing; Master’s Degree preferred.
  • Experience: Minimum of 7 years of experience with 2-3 years as Director, Case Management.
  • Certifications: Certified Case Management (CCM) or Accredited Case Manager (ACM)
  • Licenses: Current licensure as a Registered Nurse in state of residence.
  • Ability to travel up to 50%
