About the role

Key responsibilities & impact
  • Evaluate, plan, direct, and administer physical therapy programs
  • Assess patient condition based on referring physician diagnosis
  • Develop and update plans of care in collaboration with patients and families
  • Deliver prescribed exercise, hands-on care, patient education, therapeutic exercises, manual techniques, functional mobility training, activities of daily living training, and modalities
  • Reassess patients and modify treatment plans as needed
  • Communicate patient progress and physical limitations to supervisors, physicians, and team members
  • Provide age-specific and population-specific care using a developmental and patient/family-directed approach
  • Establish treatment plans with measurable goals and time frames
  • Provide cost-effective care according to billing guidelines
  • Complete documentation in accordance with policies, standards, and regulatory requirements
  • Provide supervision as required

Requirements

What you’ll need
  • Bachelor’s Degree in Physical Therapy
  • Sound clinical reasoning in synthesizing evaluation data and identifying treatment diagnoses
  • Knowledge of physical therapy techniques
  • Ability to provide safe, compassionate, and competent physical therapy treatments
  • Ability to communicate with patients, families, and co-workers in a friendly, supportive manner
  • Ability to travel to patient care assignments for Home Care only
  • American Heart Association Basic Life Support certification, State of Pennsylvania, upon hire
  • Licensed Physical Therapist, State of Pennsylvania, upon hire
  • Ability to lift and carry 40–50 lbs.
  • Frequent to continuous standing and walking
  • Ability to perform role-specific activities including kneeling, bending, squatting, and CPR as required
