Behavioral Health Case Manager
Lehigh Valley Health NetworkBehavioral Health Case Manager coordinating mental health care with patients and primary care providers at Lehigh Valley Health Network. Providing case management and evidence-based interventions for effective mental healthcare.
About the role
Key responsibilities & impact- Coordinates and supports mental health care to patients enrolled in Collaborative Care Management (CoCM).
- Provides evidence based, short term interventions, case management needs, and acts as a core member of the CoCM team.
- Screens and assesses patients for common mental health needs, monitoring treatment efficacy.
- Communicates recommendations to Primary Care Providers.
- Tracks/document patient follow-up and clinical outcomes in an EPIC based registry.
- Completes relapse prevention plans with patients in remission.
Requirements
What you’ll need- Bachelor’s Degree in social work, psychology, counseling, nursing or other related mental health field.
- 2 years of experience in mental health/behavioral health experience or related setting.
- Formal education or specialized training in behavioral health.
- Demonstrated knowledge and experience in a behavioral health field.
- Ability to work effectively with people from a broad range of backgrounds.
- Must possess a high degree of emotional intelligence.
- Knowledge of evidence-based interventions such as Motivational Interviewing, Problem Solving Techniques, and/or Behavioral Activation.
