About the role
- Evaluate, plan, direct, and administer physical therapy programs to restore function, prevent disability, and help patients reach maximum independence
- Assess patient condition based on diagnosis from referring physician and develop/update plan of care in collaboration with patient/family
- Implement individualized treatment plans using therapeutic exercises, manual techniques, functional mobility training, activities of daily living training, and modalities
- Perform reassessment when indicated and modify physical therapy plans accordingly
- Establish treatment plans with measurable goals and time frames and provide cost effective care per departmental billing guidelines
- Communicate patient progress and physical limitations to supervisor, physicians, and other team members
- Provide supervision in accordance with regulatory requirements as appropriate
- Complete and submit all documentation based on policies/standards and regulatory requirements
- Provide patient education to improve quality of life and assist in returning patients to prior level of function
- Work schedule: M-F 8-4:30 with one 8-hour weekend shift and 1-2 holidays per year; Day Shift; Onsite
Requirements
- Bachelor’s Degree Physical Therapy
- American Heart Association Basic Life Support - State of Pennsylvania (Upon Hire)
- PT - Licensed Physical Therapist - State of Pennsylvania (Upon Hire)
- Demonstrates sound clinical reasoning in synthesizing data gathered in the evaluation and identifying treatment diagnoses
- Knowledge of physical therapy techniques
- Must be able to provide safe, compassionate, and competent physical therapy treatments
- Must be able to communicate with patients, family, and co-workers in a friendly, supportive manner
- Ability to travel to patient care assignments - Home Care only
- Preferred: Master’s Degree Physical Therapy or Doctorate Physical Therapy
- Preferred: 1 year clinical experience for Home Care
- Physical demands: lift and carry 40-50 lbs; push/pull patients (requires 29 lbs push force); lateral transfers up to 50 lbs; frequent to continuous standing/walking; kneeling, bending, squatting; perform CPR
