Therapist

• Evaluate, plan, direct, and administer physical therapy programs to restore function, prevent disability, and help patients reach maximum independence
• Assess patient condition based on diagnosis from referring physician and develop/update plan of care in collaboration with patient/family
• Implement individualized treatment plans using therapeutic exercises, manual techniques, functional mobility training, activities of daily living training, and modalities
• Perform reassessment when indicated and modify physical therapy plans accordingly
• Establish treatment plans with measurable goals and time frames and provide cost effective care per departmental billing guidelines
• Communicate patient progress and physical limitations to supervisor, physicians, and other team members
• Provide supervision in accordance with regulatory requirements as appropriate
• Complete and submit all documentation based on policies/standards and regulatory requirements
• Provide patient education to improve quality of life and assist in returning patients to prior level of function
• Work schedule: M-F 8-4:30 with one 8-hour weekend shift and 1-2 holidays per year; Day Shift; Onsite
