Clinical Pharmacist – Population Health
Lehigh Valley Health NetworkClinical Pharmacist coordinating pharmacy initiatives for high-risk patients in a leading health network. Engaging in performance improvement projects to enhance medication therapy management and health outcomes.
About the role
Key responsibilities & impact- Coordinates pharmacy initiatives involving registry or high-risk patients identified within the LVPHO (LVPG, MATLAV and Affiliate) practices
- Identifies, organizes, and completes performance improvement projects related to medication use
- Develops and implements pharmacy initiatives directed at promoting high value, high quality, safe, and cost-efficient medication therapy management in accordance with value-based initiatives
- Applies subject matter expertise in clinical pharmacy, therapeutic alternatives, utilization management, medication adherence strategies, and Medication Therapy Management to promote positive outcomes in LVHN value-based reimbursement contract programs
- Critically reviews drug utilization data provided by insurers and analytic team and assure ongoing performance improvement of the pharmaceutical care services provided
- Evaluates patient EMR profiles for appropriateness of medication therapy, conducts patient appointments, and provides and documents patient drug therapy recommendations
- Identifies prescribing patterns that may lead to improper drug selection, sub-therapeutic dosage, overdose, patient failure to receive medication, adverse drug reactions, drug interactions, and medication without indication and implement strategies to resolve cost barriers
- Interact with patient, providers, and pharmacy to positively impact CMS STARS, PQA, or HEDIS quality measures
- Works with Population Health Director and ICC Pharmacy Manager to help implement strategic population health pharmacy plans
- Provides education to providers and staff via presentations, newsletters, formal utilization reviews, and one-on-one interactions
- Ensures quality of care through formulary and/or medication management and quality improvement projects
Requirements
What you’ll need- Bachelor’s Degree Pharmacy or Doctorate Pharmacy
- 2 years relevant pharmacist experience in patient care or population health or 1 year PGY-1 Residency
- Skilled in critical appraisal of medical literature
- Capable of independently reaching therapeutic decisions in patient care and communicating them to physicians and other involved health professionals
- Ability to accurately monitor medications
- Ability to make independent judgments under stressful situations at times
- Identify, organize, and complete performance improvement projects related to medication management
- Ability to work effectively in a team environment
- Knowledge of laws, regulations, and accreditation standards related to medication management
- RP - Registered Pharmacist - State of Pennsylvania Upon Hire
