About the role
Key responsibilities & impact- Serve as the main liaison between the patient, family, and clinical team
- Coordinate, navigate and schedule care, including outpatient, procedures, and diagnostics
- Support pre-visit planning for a smooth operation in busy practices
- Manage complex appointment scheduling across care continuum
- Conduct patient outreach for follow-up on care and appointment needs
- Create and maintain patient records and manage referral requirements
- Facilitate communication amongst the patient, provider, and care team
Requirements
What you’ll need- High School Diploma/GED
- 2 years experience interacting with patients performing core functions
- 3 years experience in a complex health system environment or related field
- High degree of accuracy and attention to detail
- Ability to handle numerous tasks simultaneously
- Working knowledge of basic computer functionality
- Effective and professional written and verbal communication
- Ability to maintain strict confidentiality
- Critical thinking skills
