About the role
- Evaluates, plans, directs, and administers physical therapy programs to restore function
- Provides assessment of patient condition based on diagnosis from the referring physician
- Develops and updates an appropriate plan of care in collaboration with the patient/family
- Implements an individualized treatment plan through skilled, evidence-based therapy techniques
- Communicates patient progress and physical limitations to supervisors and team members
Requirements
- Bachelor’s Degree in Physical Therapy
- American Heart Association Basic Life Support - State of Pennsylvania Upon Hire
- PT - Licensed Physical Therapist - State of Pennsylvania Upon Hire
- 1 year clinical experience for Home Care (preferred)
- Demonstrates sound clinical reasoning and knowledge of physical therapy techniques
- Ability to travel to patient care assignments - Home Care only
