About the role
- Provides mental health assessment, diagnosis, treatment, and crisis intervention services for adult and/or child members who present with a broad range of mental health needs.
- Coordinates care for patients and assists them in accessing services and resources that will help maintain them in the community in the least restrictive environment.
- Oversees all utilization management activity to include authorization of services as well as management of denial and appeal process.
- Ensures all patient discharge planning documentation is clear, concise, timely, and congruent with treatment team recommendations.
- Conducts clinical assessments for interpretation and treatment intervention with patient and families as directed by treatment plan.
- Provides case management and care coordination services for clients/patients.
- Intercedes on behalf of the individual patients to assist them in achieving and maintaining community stability.
- Acts as a liaison between patients, referral sources, and funders.
- Manages all elements of mental health - voluntary and involuntary commitment processes as regulated by Pennsylvania Mental Health Procedures Act. (chapter 5100 regs)
- Participates in quality improvement activities for the department and or program.
Requirements
- Master’s Degree Social Work, Counseling, or related field.
- Ability to remain patient and calm in stressful situations.
- Must have knowledge of community resources and understanding of how to make appropriate referrals to community and other governmental agencies for services.
- Understanding of the psychosocial/medical needs of clients in order to formulate an integrated, comprehensive treatment plan.
- LCSW - Licensed Clinical Social Worker - State of Pennsylvania.
- LPC - Licensed Professional Counselor - State of Pennsylvania.
- LSW - Licensed Social Worker - State of Pennsylvania.
