About the role
Key responsibilities & impact- Assess, plan, and deliver physical therapy services according to physician orders and the State Physical Therapy Practice Act
- Provide individualized, 1-on-1 rehabilitation treatments at Covenant Village and in patient homes throughout Rowan and Mecklenburg Counties
- Evaluate home environments, muscle strength, gait, mobility, and range-of-motion potential
- Develop tailored rehabilitative care plans
- Educate patients, families, and caregivers on home exercises, safe mobility strategies, and therapeutic appliances
- Participate in clinical conferences
- Coordinate integrated care plans with multidisciplinary teams
- Identify referrals for additional home health (PCA) services as needed
- Maintain progress summaries based on goal attainment
- Supervise Physical Therapist Assistants (PTAs) in compliance with state regulations and company standards
Requirements
What you’ll need- Degree in Physical Therapy from an accredited program
- Current Physical Therapy license in North Carolina, or eligibility to obtain it before the start date
- Excellent clinical, interpersonal, and fiscal management skills
- Proficiency with computers and electronic documentation systems
- Experienced, motivated, and compassionate Physical Therapist
