About the role

Key responsibilities & impact
  • Assess, plan, and deliver physical therapy services according to physician orders and the State Physical Therapy Practice Act
  • Provide individualized 1-on-1 rehabilitation treatments at Covenant Village and in patient homes throughout Gaston County
  • Evaluate home environments, muscle strength, gait, mobility, and range-of-motion potential
  • Build tailored rehabilitative care plans
  • Educate patients, families, and caregivers on home exercises, safe mobility strategies, and therapeutic appliances
  • Participate in clinical conferences
  • Coordinate integrated care plans with multidisciplinary teams
  • Identify referrals for additional home health PCA services
  • Maintain progress summaries based on goal attainment
  • Supervise Physical Therapist Assistants in compliance with state regulations and company standards

Requirements

What you’ll need
  • Degree in Physical Therapy from an accredited program
  • Current Physical Therapy license in North Carolina, or eligibility to obtain it before the start date
  • Excellent clinical skills
  • Excellent interpersonal skills
  • Excellent fiscal management skills
  • Proficiency with computers and electronic documentation systems
