Navigator – Managed Support

About the role
Key responsibilities & impact- Coordinating urgent supports and responding to changing situations with calm and professionalism
- Conducting needs assessments and planning support for people with complex or immediate needs
- Working alongside clinical teams, specialist services and internal teams to ensure seamless support and handovers
- Managing workflow and assessing risks in line with NASC guidelines
- Supporting quality practice through reviews, recommendations and collaborative problem solving
Requirements
What you’ll need- Experience coordinating supports and services for people with physical, intellectual or sensory disabilities
- Strong communication and relationship-building skills with the ability to connect with a diverse range of people
- The ability to stay organised and responsive while managing competing priorities
- A collaborative, team-focused approach
- Working knowledge of Te Tiriti o Waitangi and confidence applying this in practice
