About the role

Key responsibilities & impact
  • Provide social work services to patients, families, and significant others
  • Conduct psychosocial assessments related to patients’ social, emotional, and cognitive capabilities, illness, and health-maintenance needs
  • Develop and implement intervention plans addressing identified patient and family needs
  • Collaborate with multidisciplinary teams on discharge planning and care progression
  • Provide counseling and crisis intervention for diagnoses, chronic illness, palliative care, terminal care, treatment decisions, and end-of-life issues
  • Advocate for patients and families to obtain necessary services
  • Refer patients to community-based services and resources
  • Coordinate team and family conferences
  • Facilitate placement documentation, referrals, and post-acute care arrangements
  • Document assessments, interventions, and updates in the electronic medical record
  • Work with Case Managers on discharge planning
  • Identify patients and locate family members or next of kin
  • Educate patients and significant others about obtaining health insurance and welfare insurance
  • Initiate or follow up on medical assistance applications when necessary
  • Supervise MSW student field placements
  • Precept and mentor new staff members
  • Coordinate weekend discharge plans, including homecare, hospice, transportation, and post-acute facility placement
  • Perform trauma-patient assessments and develop interventions where applicable
  • Obtain post-acute authorizations and manage Livanta appeal determinations
  • Review discharge orders and identify patients appropriate for transition to the next level of care

Requirements

What you’ll need
  • Master’s Degree required
  • Graduate from an accredited Master of Social Work program required
  • Master of Social Work required
  • NJ license required
  • 2 years of hospital-based social work experience
  • Knowledge of medical terminology and disease processes
  • Knowledge of community resources
  • Knowledge of insurance regulations and requirements
  • Ability to work with multidisciplinary teams, outside resources, patients, families, and significant others
  • Ability to establish an appropriate plan to safely discharge patients to the next level of care
  • Ability to lift and carry 25 pounds
  • Frequent sitting and standing
  • Frequent keyboard use
  • Patient-care providers may be required to kneel, bend, squat, and perform CPR
  • Able to float to all three locations: Stratford, Washington Township, and Cherry Hill
  • National Association of Social Workers affiliation listed
