About the role

Key responsibilities & impact
  • Provide social work services and conduct psychosocial assessments for patients and significant others
  • Assess and enhance patients’ and families’ ability to cope with illness, convalescence, discharge, and environmental problems
  • Develop and implement intervention plans addressing identified needs
  • Collaborate with multidisciplinary healthcare teams on discharge planning and care progression
  • Provide counseling, psychosocial interventions, crisis interventions, and education to patients and families
  • Advocate for patients and families to obtain necessary services and community resources
  • Coordinate team and family conferences
  • Address cases involving abuse, domestic violence, guardianship, foster care, adoption, psychiatric placement, advance directives, trauma, and substance abuse
  • Complete placement-related documentation, referrals, insurance and medical assistance applications, and electronic medical record updates
  • Work with Case Managers to provide discharge planning services
  • Identify patients and locate family members or next of kin
  • Supervise MSW student field placements and mentor new staff
  • Facilitate weekend discharge plans, including homecare, hospice, transportation, and post-acute placements
  • Perform trauma assessments and interventions when applicable
  • Obtain post-acute authorizations and manage Livanta appeal determinations
  • Review discharge orders and coordinate transitions to the next level of care
  • Float to all three locations: Stratford, Washington Township, and Cherry Hill

Requirements

What you’ll need
  • Master’s Degree required
  • Graduate from an accredited Master of Social Work program required
  • Master of Social Work required
  • New Jersey license required
  • 2 years of hospital-based social work experience
  • Knowledge of medical terminology and disease processes
  • Knowledge of community resources
  • Knowledge of insurance regulations and requirements
  • Ability to work with multidisciplinary teams, outside resources, patients, families, and significant others
  • Ability to establish an appropriate plan to safely discharge patients to the next level of care
  • Ability to lift and carry 25 lbs.
  • Frequent sitting and standing
  • Frequent keyboard use
  • Patient-care providers may need to kneel, bend, squat, and perform CPR
  • National Association of Social Workers affiliation listed
