About the role
Key responsibilities & impact- Coordinate comprehensive care for a defined patient population throughout the continuum of care
- Coordinate all aspects of discharge planning and establish safe discharge plans
- Advocate for patients and families throughout the episode of care
- Facilitate communication among the healthcare team and patients/families
- Facilitate patient progression during hospital stays
- Review length-of-stay and barriers data during multidisciplinary rounds and escalation/outlier meetings
- Review patient status daily to identify issues requiring priority intervention that may affect discharge plans
- Facilitate or support daily patient Plan of Care Huddle Rounds as Patient Progression Leader
- Monitor patient progress and intervene to promote efficient progression
- Identify and resolve barriers to patient care and discharge
- Escalate unresolved barriers to department leadership
- Interact with co-workers, visitors, and staff consistent with Jefferson's values
Requirements
What you’ll need- Bachelor's Degree of Nursing
- 3 years of case management or discharge planning experience
- Effective use of conflict resolution skills
- Interdisciplinary collaborative approach to patient care planning and discharge coordination
- Understanding of developmental stages and their impact on adjustment to illness/trauma
- Ability to identify adolescent behaviors related to body image, independence, sexual identity, identity development, and illness severity
- Understanding of end-of-life issues
- Effective communication skills with neonate, infant, adolescent, and geriatric patients
- Ability to recognize signs of physical and/or emotional abuse or neglect
- Ability to identify hearing and sight deficiencies in geriatric populations
- Licensed Registered Nurse in the State of New Jersey
- Ability to lift and carry 25 lbs.; frequent sitting, standing, and keyboard use; may require kneeling, bending, squatting, and CPR
