About the role
Key responsibilities & impact- Collaborates across interdisciplinary teams to advance health system strategies
- Partners with clinical, operational, and financial stakeholders to align care progression workflows
- Oversees population to ensure timely progression of care and safe discharge planning
- Identifies patient clinical conditions, testing and interventions in the ambulatory space
- Monitors medical necessity, reduces avoidable days, and resolves barriers to care in collaboration with physicians
- Collaborates in the development and implementation of performance improvement activities
- Monitors length of stay, readmissions, and throughput metrics; identifies trends
Requirements
What you’ll need- Bachelor’s Degree Nursing
- 5 years experience in-patient acute care hospital
- RN - Licensed Registered Nurse_NJ - State of New Jersey Upon Hire
- Knowledge of care and utilization management
- Strong interpersonal and organizational skills to manage case load and communicate with multi-disciplinary team members
- Ability to work independently and set priorities to ensure appropriate use of hospital resources
- Ability to lead individuals and groups
