Key responsibilities & impact

Collaborates across interdisciplinary teams to advance health system strategies
Partners with clinical, operational, and financial stakeholders to align care progression workflows
Oversees population to ensure timely progression of care and safe discharge planning
Identifies patient clinical conditions, testing and interventions in the ambulatory space
Monitors medical necessity, reduces avoidable days, and resolves barriers to care in collaboration with physicians
Collaborates in the development and implementation of performance improvement activities
Monitors length of stay, readmissions, and throughput metrics; identifies trends

Requirements

What you’ll need

Bachelor’s Degree Nursing
5 years experience in-patient acute care hospital
RN - Licensed Registered Nurse_NJ - State of New Jersey Upon Hire
Knowledge of care and utilization management
Strong interpersonal and organizational skills to manage case load and communicate with multi-disciplinary team members
Ability to work independently and set priorities to ensure appropriate use of hospital resources
Ability to lead individuals and groups
