About the role
- Advocates for the patient and family throughout the entire episode of care.
- Monitors patient’s progression intervening as needed to ensure efficient patient progress.

Requirements
- BSN
- 2 years of medical surgical nursing experience
- 1-2 years of acute care, homecare or population health RN case management experience
- CCM certified
