About the role
Key responsibilities & impact- Conduct comprehensive reviews of patient care documentation according to established standards, protocols, and regulatory requirements
- Complete and submit accurate Minimum Data Set (MDS) assessments within required timelines
- Develop and update individualized care plans for residents
- Ensure assessments and documentation align with Medicare/Medicaid, CMS, and state regulations
- Collaborate with nurses, therapists, clinical staff, managers, and other healthcare stakeholders to support quality patient care
- Maintain precise documentation to support reimbursement and compliance
- Apply knowledge of PDPM (Patient-Driven Payment Model) to support accurate reimbursement classification
- Review patient records and clinical documentation for completeness and accuracy
- Identify missing, incomplete, or incorrect information and follow up accordingly
- Provide feedback and support ongoing education and training based on documentation review findings
- Manage multiple deadlines independently in a remote work environment
Requirements
What you’ll need- Current and valid PHRN license
- Must have bedside/clinical experience
- Must have at least 2 years experience with MDS (Minimum Data Set)
- Extensive team lead / supervisory experience
- Experience supporting a U.S. Skilled Nursing Facility (SNF)
- Strong knowledge of prehospital patient care protocols, standards, and regulatory requirements
- Excellent attention to detail and analytical skills
- Effective communication and interpersonal abilities
- Ability to work independently and collaboratively in a team-oriented environment
- Commitment to continuous learning and professional development
- Willingness to work on a shifting schedule
