About the role
- Conduct intake sessions and individual assessments
- Develop and monitor treatment plans
- Provide therapy services and connect patients with essential community resources
- Document patient services in electronic health records accurately and in real-time
- Use standardized assessment tools and exhibit strong diagnostic skills
- Participate in quality improvement initiatives and collaborate with a multidisciplinary team
- Support program designed to fill gaps in the outpatient continuum for hospitals and health systems
Requirements
- 2+ years of experience as an independently Licensed Clinical Social Worker or Licensed Professional Counselor
- Active LCSW or LPC license in Wisconsin required
- Proven experience in both intake and therapy, with at least 2 years of diagnostic experience in these settings
- Experience with group therapy strongly preferred
- Full-time availability (40 hours per week) during CST hours
- Experience & willingness to treat patients across various age groups, ideally all ages but a minimum of ages 13+
- Must reside in the U.S. or a U.S. territory
- Have passed the ASWB, NCW, and/or NCMHCE examination to gain clinical licensure (mandatory requirement)
