About the role

Key responsibilities & impact

Provides psychosocial assessments
Provides crisis intervention services and counseling to patients
Coordinates healthcare services throughout the continuum of care on behalf of the patient
Assists in discharge planning
Conducts thorough psychosocial assessment of patient
Provides assistance with evaluation and referral in situations of possible abuse/neglect
Implements social service interventions based on psychosocial assessment
Counsels patients/family in adjustment to illness, treatment and grieving/conflict resolution
Assists patients/family members in obtaining advanced directives and durable power of attorney for health care
Works collaboratively with Case Managers and the health care team
Acts as liaison between patients, family members, physicians, interdisciplinary team and external agencies

Requirements

What you’ll need

Masters in social work (MSW) from an accredited school
Must have a current Oklahoma State Driver's License
Must have a driving record which is acceptable to our insurance carrier
