About the role

Key responsibilities & impact
  • Provides psychosocial assessments
  • Provides crisis intervention services and counseling to patients
  • Coordinates healthcare services throughout the continuum of care on behalf of the patient
  • Assists in discharge planning
  • Conducts thorough psychosocial assessment of patient
  • Provides assistance with evaluation and referral in situations of possible abuse/neglect
  • Implements social service interventions based on psychosocial assessment
  • Counsels patients/family in adjustment to illness, treatment and grieving/conflict resolution
  • Assists patients/family members in obtaining advanced directives and durable power of attorney for health care
  • Works collaboratively with Case Managers and the health care team
  • Acts as liaison between patients, family members, physicians, interdisciplinary team and external agencies

Requirements

What you’ll need
  • Masters in social work (MSW) from an accredited school
  • Must have a current Oklahoma State Driver's License
  • Must have a driving record which is acceptable to our insurance carrier
