About the role

Key responsibilities & impact

Provides psychosocial assessments, crisis intervention services and counseling to patients
Coordinates healthcare services throughout the continuum of care on behalf of the patient
Assists in discharge planning
Conducts thorough psychosocial assessment of patient (including emotional, cultural and financial needs)
Provides assistance with evaluation and referral in situations of possible abuse/neglect for adult and pediatric patients
Implements social service interventions based on psychosocial assessment
Counsels patient/family in adjustment to illness, treatment and the process of grieving/conflict resolution
Assists patients/family members in obtaining advanced directives and/or durable power of attorney for health care
Works collaboratively with Case Managers and health care team in facilitating patient hospital discharge plans
Acts as a liaison between patients, family members, physicians, interdisciplinary team and external agencies
Maintains and submits all required documentation in a timely manner
Dictates psychosocial history, and initiates the development of treatment plans

Requirements

What you’ll need

Masters in social work (MSW) from an accredited school
Must be able to communicate effectively in English (verbal/written)
Must have a current Oklahoma State Driver's License with an acceptable driving record for insurance
