About the role

Key responsibilities & impact
  • Provides psychosocial assessments, crisis intervention services and counseling to patients
  • Coordinates healthcare services throughout the continuum of care on behalf of the patient
  • Assists in discharge planning
  • Conducts thorough psychosocial assessment of patient (including emotional, cultural and financial needs)
  • Provides assistance with evaluation and referral in situations of possible abuse/neglect for adult and pediatric patients
  • Implements social service interventions based on psychosocial assessment
  • Counsels patient/family in adjustment to illness, treatment and the process of grieving/conflict resolution
  • Assists patients/family members in obtaining advanced directives and/or durable power of attorney for health care
  • Works collaboratively with Case Managers and health care team in facilitating patient hospital discharge plans
  • Acts as a liaison between patients, family members, physicians, interdisciplinary team and external agencies
  • Maintains and submits all required documentation in a timely manner
  • Dictates psychosocial history, and initiates the development of treatment plans

Requirements

What you’ll need
  • Masters in social work (MSW) from an accredited school
  • Must be able to communicate effectively in English (verbal/written)
  • Must have a current Oklahoma State Driver's License with an acceptable driving record for insurance
