About the role
- Lead, mentor, and support a team of care managers to deliver comprehensive care coordination and case management services.
- Monitor and evaluate care plans to ensure they meet member needs and comply with clinical guidelines and regulatory standards.
- Facilitate communication and collaboration among care managers, healthcare providers, members, and families to optimize care delivery.
- Conduct regular performance reviews, provide feedback, and identify training needs to promote continuous professional growth.
- Analyze care management data and outcomes to identify trends, address challenges, and implement quality improvement initiatives.
Requirements
- Bachelor’s degree in Nursing, Social Work, Healthcare Administration, or a related field.
- Minimum of 3 years of experience in care management or case management within a healthcare or social assistance setting.
- At least 1 year of supervisory or team leadership experience in a healthcare environment.
- Strong knowledge of care coordination principles, healthcare regulations, and member privacy laws (e.g., HIPAA).
- Proficiency in electronic health records (EHR) systems and care management software.
- Relevant experience may substitute for the educational requirement on a year-for-year basis.
