About the role

Key responsibilities & impact

Provide care coordination and management for families of children 0-8 with developmental delays.
Provide family-centered culturally competent care coordination and social work services for children and their families, birth to age eight.
Compile necessary information and data for programming and accurate reports and billing for various funding sources.
Conduct other clerical work and administrative duties as required.

Requirements

What you’ll need

Bachelor's Degree in related field from an accredited college/university required.
Licensed Social Worker/Ability to become LSW or LCSW preferred.
Pediatric experience with birth to eight populations is preferred.
