Director of Community Health Worker Services – Georgia

• Your team will use IMPaCT’s evidence-based infrastructure to help customer organizations plan, build and support Community Health Worker programs at enterprise scale.
• You will oversee implementation of customer journeys across the following stages: Strategic planning conversations during which your team will help customers articulate their vision for a Community Health Worker program and then work backwards to an operational plan.
• Your team will conduct gap analyses of any existing Community Health Worker programs, define key performance metrics, build financial models, identify priority populations, and map workflows, including the flow of information between the electronic health record and IMPaCT application.
• Once you have aligned on a plan, you and your team will help your client execute it; you will use IMPaCT’s evidence-based infrastructure to hire and train empathic, trustworthy Community Health Workers who come from within the communities they serve.
• You will partner with our technology team to help organizations optimize their use of IMPaCT’s cloud-based application, including supporting electronic health record integration and billing pathways.
• You will stay closely engaged as your client builds their Community Health Worker program, meeting with them biweekly and coaching them on topics ranging from enrollment to clinical integration to billing.
• You will use IMPaCT’s technology and analytics to run continuous improvement cycles; you will ensure that your client’s Community Health Worker program is delivering magic with consistency at scale.
• You will manage a growing team and create systems that ensure effective performance and account management across the team.
• You will be accountable for Key Performance Indicators around Customer Success (e.g., customer delight, customer retention).

• What You’ll Do: Project & Relationship Excellence: Do what you say you will: Delight your customers by keeping your commitments and proactively communicating when deliverables are at risk or behind schedule. Document like a pro: Capture meeting notes with laser focus, highlighting decisions/actions (e.g., Per 6/15 discussion, Sunbelt will pilot new workflows by Q3). Communicate with polish: Deliver succinct, actionable updates (e.g., quarterly business review decks, risk mitigation plans).
• Customer Success Management: Co-manage a portfolio of 10-15 healthcare customers, ensuring retention and contract expansion through tight execution and relationship management. Support end-to-end CHW program launches, implementation, and ongoing success, coordinating across stakeholders to ensure successful execution and achievement of customer goals. Own and streamline the Tech Review process by collaborating with IT and customer stakeholders to assess technical needs, align on compliance requirements, and establish clear timelines and success metrics—ensuring frictionless platform adoption and scalability.

• As a Physician Partner, you will: Dedicate 5–10 hours per week to real-time telehealth visits with patients referred by CHWs including consults, prescribing medications when appropriate, and providing clinical guidance. • Collaborate with CHWs to evaluate patient needs and determine eligibility for Community Health Integration services. • Provide clinical consultation and oversight to CHWs as they support patients with social needs and healthcare navigation. • Promote preventive care and overall well-being through education and evidence-based strategies. • Maintain accurate and up-to-date patient records, including reviewing and affirming the documentation created by the patient’s CHW. • Respond to patient questions, chart notes, and administrative messages as they transition to a primary care home. • Provide occasional follow-up visits to address urgent or time-sensitive medical needs. • Complete required training and adhereto company policies and best practices.
