About the role

Key responsibilities & impact
  • Provide operational leadership and oversight for care coordination services
  • Direct, coach, and develop supervisor-level leaders through performance management, accountability, succession planning, and leadership development
  • Establish and monitor productivity, quality, service, and operational performance metrics
  • Use data to identify opportunities and implement improvements
  • Partner with clinical, operational, physician, and analytics leadership to align services with organizational priorities and population health goals
  • Lead implementation and sustainment of standardized workflows, operational best practices, and continuous improvement initiatives
  • Manage staffing, resource allocation, workload distribution, and operational capacity planning
  • Identify operational barriers, process variation, and performance risks and implement solutions
  • Lead operational change initiatives, including new technologies, workflows, programs, and processes
  • Collaborate with Network Operations, Care Management, Medical Group leadership, Analytics, and affiliate practice stakeholders

Requirements

What you’ll need
  • Associate degree or equivalent experience is required; relevant experience may be considered in place of formal education
  • At least three years of experience in healthcare operations, care coordination, care management, population health, clinical operations, practice transformation, or a related healthcare operations or leadership environment
  • Experience leading teams, projects, operational initiatives, or workflow improvement efforts
  • Experience using operational metrics, reports, and performance data to guide decisions and improve outcomes
  • Strong verbal and written communication skills
  • Strong computer skills, including Microsoft Office applications and other applicable operational systems
  • Leadership experience managing supervisors or other people leaders preferred
  • Experience in care coordination, care management, population health, value-based care, ambulatory operations, or patient outreach programs preferred
  • Experience leading process improvement and operational transformation initiatives preferred
  • Experience with Lean principles, continuous improvement methodologies, or workflow standardization preferred
  • Experience collaborating with physicians, clinical leaders, operations leaders, analytics teams, and cross-functional stakeholders preferred
