About the role

Key responsibilities & impact
  • Coordinates and manages the care of patients with chronic or complex conditions
  • Works collaboratively with physicians, interdisciplinary teams, individual patients, and families
  • Performs comprehensive care continuum process including assessment, planning, implementation, coordination, monitoring, and evaluation
  • Advocates for, educates, and coaches patients, families, and caregivers on treatment options
  • Supports goal-setting and development of self-care skills
  • Facilitates communication and coordination among members of the healthcare team

Requirements

What you’ll need
  • Current Registered Nurse (RN) license in state of practice
  • Bachelor of Science in Nursing (BSN) from an accredited institution (degree verification required)
  • Demonstrated clinical nursing experience, and familiarity with patient-related terminology and processes
  • Demonstrated experience in care management
  • Certified Case Manager (CCM) or other relevant certification preferred
  • Two (2) years of experience in nursing, preferably in primary care or ambulatory care settings
  • One (1) year of experience in care management
  • Experience in chronic disease management
  • Knowledge of value-based care models and principles
