About the role
Key responsibilities & impact- Utilizes clinical expertise to perform psychosocial assessments
- Develops and implements care plans in collaboration with the care team
- Assesses crisis situations to provide clinical counseling and resources
- Provides individual and family treatment as indicated
- Works collaboratively with patients, their support persons, healthcare providers, and community resources
- Develops and monitors care plans addressing social determinants of health
- Promotes mental health care including women’s services and trauma-informed care
- Facilitates clinical goals of care discussions with patients and families
Requirements
What you’ll need- Master of Social Work (MSW) from an accredited institution
- Current state licensure, as applicable (CSW/LCSW)
- Basic computer proficiency with word processing and spreadsheet software
- Strong written and verbal communication skills
- Demonstrated ability to apply critical thinking skills
- Caregivers must maintain current BLS certification, have a current driver’s license, current auto insurance, acceptable driving record, and reliable transportation
- Case Management Certification (preferred)
- Experience in clinical care management or social work (preferred)
- Understanding of care management principles and practices (preferred)
- Understanding of health insurance products and related processes (preferred)
