About the role

Key responsibilities & impact

Utilizes clinical expertise to perform psychosocial assessments
Develops and implements care plans in collaboration with the care team
Assesses crisis situations to provide clinical counseling and resources
Provides individual and family treatment as indicated
Works collaboratively with patients, their support persons, healthcare providers, and community resources
Develops and monitors care plans addressing social determinants of health
Promotes mental health care including women’s services and trauma-informed care
Facilitates clinical goals of care discussions with patients and families

Requirements

What you’ll need

Master of Social Work (MSW) from an accredited institution
Current state licensure, as applicable (CSW/LCSW)
Basic computer proficiency with word processing and spreadsheet software
Strong written and verbal communication skills
Demonstrated ability to apply critical thinking skills
Caregivers must maintain current BLS certification, have a current driver’s license, current auto insurance, acceptable driving record, and reliable transportation
Case Management Certification (preferred)
Experience in clinical care management or social work (preferred)
Understanding of care management principles and practices (preferred)
Understanding of health insurance products and related processes (preferred)
