Key responsibilities & impact

The Nurse Care Manager coordinates and manages the care of patients with chronic or complex conditions.
Works collaboratively with physicians, interdisciplinary teams, individual patients, and families to promote positive outcomes and ensure continuity of care.
Performs a comprehensive care continuum process that includes assessment, planning, implementation, coordination, monitoring, and evaluation of the services and options required to support each individual's health needs and goals.
Utilizes skilled communication, education, and available resources to promote high-quality, cost-effective outcomes.
Key responsibilities include coordinating services, addressing barriers to care, and optimizing resource allocation while balancing clinical quality and cost management for an assigned patient population.

Requirements

What you’ll need

Current Registered Nurse (RN) license in state of practice.
Bachelor of Science in Nursing (BSN) from an accredited institution (degree verification required).
Demonstrated clinical nursing experience, and familiarity with patient-related terminology and processes.
Demonstrated experience in care management.
