About the role
Key responsibilities & impact- Identify resources and community support available to families or caregivers
- Provide information, referrals, and non-clinical assistance linking families to medical, psychosocial, and basic-needs services
- Empower families to advocate for themselves through education, guidance, and supportive tools
- Assist with accessing programs and supports addressing psychosocial needs within Imagine Pediatrics and the broader community
- Advocate with community agencies on behalf of clients and families to reduce service gaps and barriers
- Connect with internal staff and providers to support system navigation, including eligibility, pediatrics, behavioral health, and public health
- Help develop an internal community resource library, excluding clinical library materials
- Link families to community-based and school-based services
- Network with community resources to increase knowledge of available services
- Administratively document memoranda of understanding for outside community-agency partnerships
- Develop effective working relationships with interdisciplinary care teams
- Participate in scheduled interdisciplinary consultations regarding administrative and non-clinical resources
- Perform other duties as assigned
Requirements
What you’ll need- Bachelor’s degree or higher in healthcare, behavioral health, or social services related field preferred
- Certification as a Community Health Worker highly desired
- Familiarity with navigating social service agencies
- Reliable transportation
- Valid Driver's License
- Ability to produce a copy of current auto insurance certificate
- Experience with resourcing and telephonic services
- Bilingual Spanish preferred
- Availability to work 8am-5pm Monday-Friday
