About the role
Key responsibilities & impact- Accept incoming and return patient phone calls involving clinical inquiries, perform protocol-based triage as appropriate, and route phone notes to providers through the EHRS
- Place calls and create template-based letters regarding lab results for timely delivery to patients
- Review daily schedules and complete pre-visit preparations, ensuring lab, imaging, consultation, care transition, and health-status information is available in medical records
- Contact patients, consultants, imaging centers, and hospitals to obtain missing information
- Call at-risk no-show patients and new patients for onboarding visits; remind them of responsibilities, assess barriers to care, and initiate social service referrals
- Initiate disability forms, home health orders, durable medical equipment and supply requests, and disease status letters for provider review and signature
- Assist the care team with reports to manage and assess patient population health needs
- Identify vulnerable-population patients and provide appropriate social service or community-based referrals
Requirements
What you’ll need- High-school level education or equivalency
- 2–4 years of healthcare experience in outpatient clinics
- Experience in specialty/multispecialty clinics strongly preferred
- Spanish fluency preferred
- Ability to work remotely on EST hours
- Availability for a 3x12-hour schedule, 7:00am–7:00pm EST, Monday–Wednesday or Wednesday–Friday
- Experience providing clinical and clerical support in a virtual value-based care environment
- Ability to perform protocol-based triage and route clinical phone notes through an EHRS
