About the role

Key responsibilities & impact

Provide direct support to patients and families in addressing social needs
Work collaboratively with community partners and care team members
Identify resources and community support available to the family or caregivers
Provide information, referrals, and non-clinical assistance with linkage to medical, psychosocial, and basic needs services
Empower families to advocate for themselves by providing education, guidance, and supportive tools to navigate systems and access needed services
Advocate with community agencies for, and on behalf, of clients and families
Assist in development of an internal community resource library
Connect with relevant internal staff and providers to assist with system navigation
Network with new and established community resources to increase knowledge of services

Requirements

What you’ll need

Bachelor’s degree or higher in healthcare, behavioral health, or social services related field preferred
Certification as a Community Health Worker highly desired
Familiar with navigating social service agencies
Must have reliable transportation, a valid Driver's License, and ability to produce a copy of current auto insurance certificate
Experience with resourcing and telephonic services
Bilingual Spanish preferred
