About the role

Key responsibilities & impact

Requirements

What you’ll need
  • Master's degree in a Behavioral Health or related field
  • Active, unrestricted Kentucky licensure as an LCSW, LPCC, LMFT, or other independent Kentucky behavioral health license with at least 1 year of post-degree clinical experience; alternatively, an active, unrestricted Kentucky RN license with a bachelor's degree in nursing and at least 3 years of behavioral health experience
  • 2+ years of case management or care management experience
  • Experience working with adult populations
  • Up to 75% travel; approximately 30–40 miles or 40 miles per day
  • Monday through Friday availability, 8:00 AM–5:00 PM, with flexibility to work overtime as needed
  • Valid state driver's license
  • Maintain personal vehicle liability insurance meeting the residing state's minimum limits or the specified $25,000/$25,000/$10,000 limits, whichever is higher
  • Patient-facing role requiring TB screening
  • Preferred: experience with Kentucky Medicaid, managed care, SPMI and/or SUD, high-risk populations, behavior change, health promotion, wellness coaching, and member engagement
  • Preferred: Certified Case Manager (CCM) certification
  • Preferred: knowledge and application of motivational interviewing and trauma-informed care
  • Preferred: bilingual English and Spanish, including speaking, reading, writing, interpreting, and explaining documents in Spanish
