About the role

Key responsibilities & impact

Assess and evaluate members' needs and requirements to achieve or maintain optimal wellness
Guide members and families toward appropriate care and wellbeing resources
Facilitate interaction with community, care, and social service resources
Manage members' physical, environmental, and psychosocial health issues using varied strategies and techniques
Identify and resolve barriers to effective care
Continuously monitor care through assessments and evaluations
Ensure members progress toward desired outcomes
Create member care plans when needed
Collaborate with staff, providers, members, and families through face-to-face meetings
Travel between 75%-90% of the time to support field collaboration and member engagement

Requirements

What you’ll need

Bachelor's in nursing (BSN)
Active nursing license in South Carolina without disciplinary action
Must reside in the State of South Carolina
2 or more years of case/care management experience
2 or more years working with the behavioral health population
Knowledge of community health and social service agencies and community resources
Ability to use electronic information applications/software programs, including electronic medical records
Valid state driver's license
Personal vehicle liability insurance meeting state minimum limits or Humana's specified limits, whichever is higher
Ability to work Monday through Friday, 8:00 AM–5:00 PM Eastern Time
Ability to meet home internet requirements of at least 25 Mbps download and 10 Mbps upload
Ability to work from a dedicated space without ongoing interruptions to protect PHI/HIPAA information
