About the role

Key responsibilities & impact
  • Administer initial and ongoing long-term services and support (LTSS) assessments using person-centered thinking approaches
  • Contact members telephonically and/or in person to establish goals and priorities, evaluate resources, develop plans of care, and identify LTSS providers and community partnerships
  • Develop and continuously modify service plans and involve applicable care-team members in care planning
  • Support members with navigation of LTSS and related environmental and social needs
  • Use available member information to avoid duplicative assessments
  • Help members and caregivers access long-term services and supports, social, housing, educational, and other services
  • Assist members in maintaining Medicaid eligibility
  • Collaborate with Medical Directors, geriatricians, and care coordinators to ensure cohesive service delivery and positive member outcomes
  • Assess and evaluate member needs to promote optimal wellness and health outcomes
  • Connect members with quality services and community resources

Requirements

What you’ll need
  • Bachelor's degree in a health and human services field
  • 2 or more years of related experience
  • Knowledge of Microsoft Office Word and Excel
  • Exceptional verbal and written communication and interpersonal skills
  • Ability to use various electronic information applications and software programs
  • Ability to pass tuberculosis (TB) screening
  • Valid state driver's license
  • Maintain personal vehicle liability insurance meeting state minimums or Humana's specified limits, whichever is higher
  • Reside in Indiana
  • Have a separate room with a locked door usable as a private home office
  • Home internet with at least 25 Mbps download and 10 Mbps upload speed
  • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
  • Preferred: experience with electronic case-note documentation and multiple computer applications/systems
  • Preferred: experience with health promotion, coaching, and wellness
  • Preferred: knowledge of community health and social service agencies and community resources
