About the role

Key responsibilities & impact
  • Assess and evaluate OB members’ needs and requirements
  • Guide members and families toward appropriate care and wellbeing resources
  • Facilitate interaction with resources for member care
  • Employ strategies to manage physical, environmental, mental health/substance abuse, and psychosocial health issues
  • Identify and resolve barriers hindering effective care
  • Continuously monitor patient care through assessments and evaluations
  • Ensure patients progress toward desired outcomes
  • Create member care plans when needed
  • Make independent decisions regarding appropriate courses of action
  • Report to the Regional CM Manager

Requirements

What you’ll need
  • Reside in Florida
  • Registered Nurse license in the State of Florida with no disciplinary action
  • 3+ years of Women's Health, Obstetric, or Labor & Delivery experience
  • Comprehensive knowledge of Microsoft Office applications, including Word, Excel, and Outlook
  • Tuberculosis (TB) screening
  • AHCA II Level Background Clearance for associates working in Florida
  • Work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information
  • Minimum home internet speed of 25 Mbps download and 10 Mbps upload
  • Preferred: BSN
  • Preferred: 1+ year of Substance Use/Addiction/Behavioral/Mental Health experience
  • Preferred: Experience with case management, discharge planning, and patient education
  • Preferred: Bilingual English/Spanish or English/Creole
  • Preferred: Managed care experience
  • Preferred: Certified Case Manager (CCM)
