About the role

Key responsibilities & impact

Perform telephonic and face-to-face assessments and evaluations of member needs
Support optimal wellness by guiding members/families toward resources for care
Meet members in the field for quality assessments of their goals, needs, and barriers
Develop individualized care plans for members
Employ evidence-based strategies to manage member health
Identify and resolve barriers to effective care
Monitor member care through comprehensive assessments and evaluations
Make autonomous decisions in ambiguous situations
Follow established guidelines/procedures

Requirements

What you’ll need

Must reside in Richmond, Virginia area and be available to see members in the community
Active Registered Nurse (RN) license in the Commonwealth of Virginia without disciplinary action
2 years of prior experience in health care and/or case management
1 year of experience working with individuals who are at risk for chronic medical conditions & high social needs ( adult and pediatric populations)
Knowledge of community health and social service agencies and additional community resources
Intermediate to advanced computer skills and experience with Microsoft Word, Outlook, and Excel
Excellent keyboard and web navigation skills
Exceptional communication and interpersonal skills with the ability to quickly build rapport
Ability to travel to region-based facilities and homes for face-to-face assessments and interactions with members and/or families
