- As a Bilingual Telephonic Nurse Care Manager, you will have the opportunity to make a meaningful impact on members’ lives by supporting individuals from a variety of cultural backgrounds and communities.
- Works with an assigned member population in Illinois over the phone performing health assessments, care planning, and education on chronic conditions and any gaps in their health care plans.
- Employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues.
- Identifies and resolves barriers that hinder effective care.
- Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through use of assessment, data, conversations with member, and active care planning.
- Navigates multiple systems and uses critical thinking skills.

- Language Proficiency Testing: Any Humana associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.
- WAH Internet requirements: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required.
- HireVue: As part of our hiring process for this opportunity, we will be using an interviewing technology called HireVue.
Requirements
- Licensed Registered Nurse (RN) in the state of Illinois with no disciplinary action or ability to obtain IL RN licensure.
- Minimum of 3 years of clinical acute care experience as a licensed RN.
- Bilingual in English/Spanish, English/Polish, or English/Creole. Must be able to speak, read, and write in both languages without limitations or assistance.
- Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook.
- Bachelor's degree in Nursing (BSN) preferred.
- Experience with case management, discharge planning and patient education for adult acute care preferred.
- Managed care experience preferred.
- Certified Case Manager (CCM) preferred.
- Prior experience with Medicare & Medicaid recipients preferred.
- Experience with health promotion, coaching and wellness preferred.
- Knowledge of community health and social service agencies and additional community resources preferred.
- Workstyle: Remote, work from home.
- Typical work days/hours: Monday – Friday, 8:00am – 5:00pm CST.
