About the role
Key responsibilities & impact- Review referral requests to ensure they meet established criteria
- Ensure required data is collected and entered
- Attend meetings with referral sources as required
- Document and escalate service issues
- Monitor assigned clinicians’ work for adherence to standards and timelines
- Support the clinician network in managing expectations
- Identify efficiencies and solutions to issues
- Liaise with clients/patients, insurers, employers and healthcare professionals
- Act as the first point of contact for service providers and corporate customers
- Help manage and track complex cases
- Maintain databases and client/case files accurately and confidentially
Requirements
What you’ll need- Bachelor’s degree in a related field or equivalent experience and training
- Case Coordination and/or Case Management experience is a major asset
- Expertise in Disability Treatment and Management principles, practices and processes
- Proficiency with Microsoft Word, Excel and PowerPoint
- Excellent verbal and written English communication skills
- Effective self-directed work style with ability to work with minimal supervision
- Strong administrative and customer service skills
- Effective interpersonal skills and ability to communicate with varied stakeholders
- Ability to manage multiple demands under time constraints and deadlines
- Adaptability to changing priorities
- Ability to contribute effectively to a positive work environment
