About the role

  • Review clinical documentation for accuracy, completeness, and compliance
  • Ensure charts meet state, federal, and payer requirements
  • Conduct chart audits and identify trends, gaps, and areas for improvement
  • Collaborate with clinicians and leadership to resolve documentation issues
  • Support quality assurance and performance improvement initiatives
  • Assist with OASIS review and ensure proper documentation standards
  • Maintain timely and organized review processes to meet deadlines
  • Provide feedback and education to clinical staff as needed

Requirements

  • Active RN or LPN license required
  • Home Health experience required
  • Quality assurance or chart review experience preferred
  • Strong knowledge of home health regulations and compliance standards
  • High attention to detail and strong organizational skills
  • Ability to work independently and manage multiple priorities
  • Strong communication and problem solving skills
  • Experience with EMR systems and OASIS preferred
