About the role
- Review clinical documentation for accuracy, completeness, and compliance
- Ensure charts meet state, federal, and payer requirements
- Conduct chart audits and identify trends, gaps, and areas for improvement
- Collaborate with clinicians and leadership to resolve documentation issues
- Support quality assurance and performance improvement initiatives
- Assist with OASIS review and ensure proper documentation standards
- Maintain timely and organized review processes to meet deadlines
- Provide feedback and education to clinical staff as needed
Requirements
- Active RN or LPN license required
- Home Health experience required
- Quality assurance or chart review experience preferred
- Strong knowledge of home health regulations and compliance standards
- High attention to detail and strong organizational skills
- Ability to work independently and manage multiple priorities
- Strong communication and problem solving skills
- Experience with EMR systems and OASIS preferred
