About the role
- Review clinical documentation for accuracy, completeness, and compliance
- Ensure documentation meets state, federal, and payer requirements
- Audit charts and identify areas for improvement or correction
- Collaborate with clinicians to resolve documentation issues and provide guidance
- Support quality assurance and performance improvement initiatives
- Assist with OASIS review and documentation standards as applicable
- Maintain organized records and meet deadlines for chart review and submission
- Communicate effectively with clinical and administrative teams
Requirements
- Active RN or LPN license required
- Home Health experience required
- Experience with clinical documentation review or QA preferred
- Strong knowledge of home health regulations and compliance standards
- Attention to detail and ability to work independently
- Strong communication and problem solving skills
- Comfort working in a fast paced, deadline driven environment
- Experience with EMR systems and OASIS a plus
