About the role
- Conducting conversations with clients about their wishes for planning their end-of-life care
- Providing counseling to relatives and next of kin
- Preparing advance-care documents (e.g., advance directives/living wills) together with clients upon request
- Collaborating with physicians and other healthcare providers involved in the clients' care
- Intensive networking with local health and palliative care providers in the region
Requirements
- Completed vocational training as a caregiver for people with disabilities (Heilerziehungspfleger), educator (Erzieher), special needs pedagogue (Heilpädagoge), qualified nurse, or a degree in social pedagogy, social work, theology, psychology, or a comparable qualification
- At least three years of professional experience in the social sector
- Ideally completed further training in Health Care Planning (GVP) or willingness to undertake such training
