About the role

Key responsibilities & impact
  • Conduct thorough cardiovascular patient assessments through telehealth visits, including history reviews, physical examinations, and diagnostic tests
  • Formulate differential diagnoses and treatment plans; prescribe medications and order diagnostic tests
  • Remotely manage hypertension, congestive heart failure, arrhythmias, and coronary artery disease
  • Educate patients and families on cardiovascular disease, prevention, medication management, diet, exercise, and smoking cessation
  • Collaborate with cardiologists, PCPs, APPs, and other healthcare professionals on coordinated patient care
  • Incorporate current cardiology research and evidence-based practices into care
  • Maintain accurate patient records and ensure legal and ethical compliance
  • Advocate for patients and their care preferences
  • Participate in continuing education and quality assurance programs
  • Lead and support clinical quality initiatives, including peer review and APP and physician quality oversight
  • Develop and improve clinical quality programs and performance standards
  • Design and deliver clinician onboarding, training, and ongoing education
  • Conduct chart reviews, provide performance feedback, and promote adherence to evidence-based practices and organizational protocols
  • Serve as a clinical resource and mentor supporting patient care excellence and clinician development

Requirements

What you’ll need
  • Master’s or Doctoral degree in Nursing from an accredited Nurse Practitioner program
  • Current, unrestricted Nurse Practitioner/APRN license
  • Active national board certification, such as AANP or ANCC
  • Eligibility to obtain and maintain licensure in states required by the organization
  • Strong clinical experience as a Nurse Practitioner managing cardiovascular conditions, including hypertension, heart failure, arrhythmias, and coronary artery disease
  • Cardiology experience strongly preferred
  • Ability to independently assess patients, develop differential diagnoses, create evidence-based treatment plans, prescribe medications, and order and interpret diagnostic testing
  • Experience with clinical quality initiatives, chart review, peer review, quality assurance, or performance improvement programs
  • Experience developing or delivering clinical education, onboarding, training, or professional development for clinicians
  • Ability to provide constructive feedback, coach peers, and support clinician performance
  • Ability to serve as a clinical resource and mentor to APPs and other clinicians
  • Strong knowledge of evidence-based cardiovascular care and current clinical guidelines and research
  • Comfortable delivering care through telehealth and using virtual-care technology
  • Excellent written and verbal communication and cross-functional collaboration skills
  • Commitment to patient advocacy, education, shared decision-making, continuing education, and maintaining required credentials
