About the role
Key responsibilities & impact- Conduct home and virtual assessments for prospective and current members using the Uniform Assessment System (UAS)
- Make enrollment and continued eligibility determinations using clinical expertise and critical thinking
- Conduct assessments in skilled nursing facilities or other non-home settings as needed
- Navigate UAS software, electronic medical records, TruCare, and other technology platforms to document assessments
- Complete assessments of service needs and determine necessary adjustments
- Review prior nursing assessments and medical notes to update care plans or support nursing home admissions
- Conduct in-person Care Management visits for person-centered service planning
- Liaise between members and the CMT; assess home environments and psychosocial status
- Provide feedback to home care agencies and CMT regarding home care issues and staff performance
- Provide community-based training for frail populations, including self-care and prevention strategies
- Ensure members and responsible parties understand and agree to managed long-term care enrollment
- Perform additional duties as assigned
Requirements
What you’ll need- New York State RN license
- Ability to travel throughout downstate New York, including the five boroughs, Long Island, Rockland, and Westchester
- Prior work experience requiring technological proficiency and navigation of multiple technology platforms and modalities
- Intermediate Microsoft Word, Excel, and Outlook skills
- Time management, critical and creative thinking, communication, and problem-solving skills
- Preferred: electronic PHI database experience, especially UAS
- Preferred: field-based experience assessing, planning, and evaluating member care through home or facility visits
- Preferred experience in geriatrics, home care, discharge planning, case management, and/or medical-surgical nursing
- Knowledge of health insurance, Medicaid, Medicare, and MLTCP
- Experience working with frail adults or elderly populations and determining appropriate care plans and services
- Ability to negotiate initial service plans with members and families
- Preferred language capabilities: Spanish, Russian, French, Creole, Mandarin, or Cantonese
- Ability to handle heavy caseloads
- Preferred New York State driver’s license
