About the role
Key responsibilities & impact- Case Managers are responsible for providing client care services that include assessing treatment needs, developing, monitoring, and evaluating treatment plans and progress.
- Provide community-based case management services.
- Maintain a caseload size of 10-50 clients.
- Submit billing and payroll within set timelines and ensure all progress notes, additional required forms, and timecard are included and accurate.
- Be the first line of crisis support for clients and maintain a calm, non-defensive, supportive attitude during crisis or potential crisis situations.
- Fully participate in required supervisions, meetings, and trainings by coming prepared, actively engaging and documenting as required.
Requirements
What you’ll need- High School Diploma or Equivalent and; Certified by the Department as a Mental Health Rehabilitation Technician/Community (MHRT/C).
- Associates or Bachelor’s Degree in Social Work, Psychology, Social Services or related field preferred.
- Valid Driver’s license or HIPAA compliant transportation
