About the role
- Further development and enhancement of case management
- Establishing and optimizing structured processes between the hospital and inpatient nursing care facilities
- Coordination of discharge and admission processes
- Management of hospital discharge support
- Organizing admissions to short-term or full inpatient care
- Interface management and maintenance of professional networks
- Ensuring smooth collaboration with referrers and follow-up care providers

Requirements
- Completed vocational training or degree in healthcare or social services
- Ideally, additional qualification as a Case Manager
- Knowledge of transition processes from hospital to a care facility
- Strong professional and interpersonal skills
- Organizational and structuring skills
- Reliable, responsible personality with an independent working style
