Conduct initial intakes and assessments with children in grades K-12 and their parent/guardian
Collaborate with other Hazel Mental Health Therapists to ensure children successfully transition into our services
May provide ongoing virtual counseling and solution-focused therapeutic interventions for children in grades K-12, as needed
Provide culturally relevant and age-appropriate counseling and treatment
Collaborate with Hazel’s care management team to identify and support transition services
Maintain relationships and engage with school personnel, family members, and stakeholders
Obtain necessary state- or district-required credentialing, as requested by Hazel’s credentialing department
Manage your own caseload, ensuring timely intakes, assessments, and follow-up sessions as needed
Be responsive to communication from school personnel, families, and our care management team, and participate in occasional virtual meetings or trainings
Participate in 15 hours of paid training during initial onboarding
Hazel will credential you with insurance payors; credentialing time is reimbursable
Maintain malpractice insurance and HIPAA-compliant private workspace
Requirements
Mission-Alignment: You are inspired by providing equitable access to mental health care to students
Required: Licensed without restrictions as a Licensed Independent Social Worker (LISW), Licensed Marriage and Family Therapist (LMFT), or Licensed Mental Health Counselor (LMHC)
Required: Licensed in AZ for a minimum of 3 years
Required: Additional license in AL, CA, CO, FL, GA, HI, MA, MD, MO, NV, TX, VA, or WA; or be eligible and able to begin a state license application process immediately upon signing your contract agreement
Willingness/ability to be enrolled by Hazel with insurance companies, including Medicaid and private payers
Possess a working knowledge of treatment strategies and interventions related to the mental health needs of children and their families
Demonstrates an ability to make accurate and timely clinical judgments or recommendations
Works in collaboration with other clinical team members across Hazel Health to meet the needs of children and families
Demonstrates experience and education in all areas of clinical practice
Highly proficient in computer and technology skills, including: the ability to use a laptop, quickly learn Hazel’s clinical platform, navigate Google and Slack software applications, learn new software and programs and complete digital paperwork
Malpractice coverage must be acquired, providing a minimum of $1 million per claim and $3 million in aggregate coverage
HIPAA Compliant Workspace: You must be able to complete the functions of your job in a private, unshared and secure workspace
Equipment & Internet: Computer with webcam and microphone; stable home internet connection with at least 20 mbps; up-to-date OS and Google Chrome installed
Schedule: Availability Monday-Friday after 4pm CST and maintain schedule that accommodates student needs
