Therapist

• Provide virtual high-quality, evidence based, virtual mental health counseling, and therapeutic interventions to an assigned caseload of K-12 students
• Diagnose as needed, conduct initial mental health intake assessments, and create individual treatment plans
• Conduct clinical screeners and ongoing assessments of student progress
• Collaborate with care management team to ensure successful transitions into Hazel services and identify external transition supports
• Obtain credentialing and additional clinical state licenses as requested
• Participate in trainings, meetings, and other clinical team activities
• Complete accurate and timely documentation according to guidelines
• Maintain required weekly availability and productivity levels established by Clinical Leadership
• May provide consultative telehealth crisis support for school partners or students as assigned
